General Information on Neurotoxin for TMJ Muscle Dysfunction

The procedure involves injection of Neurotoxin into your Masseters and/or Temporalis muscles, with the intention of interrupting the stimulus for the contraction of your muscles of mastication. 


Dr Tan-Gore will carefully assess your suitability for the procedure. If muscle spasm is the cause for your symptoms, she may recommend treatment with Neurotoxin. 

The typical onset of action is 3-5 days. Dr Tan-Gore will review you at 2 weeks which is the time frame for the full effect of the treatment. 

The duration of effect varies from patient to patient but is approximately 3-6 months.

Fees

There is no Medicare Rebate for the treatment of TMJ muscle spasm available under the current guidelines.

Post Treatment Instructions

You may experience a mild headache after your treatment, especially if this is your first treatment. This headache can be managed with simple analgesia like Paracetamol and Ibuprofen

A small amount of bleeding and bruising at the injection site may occur. This settles after a short time and does not require further treatment.

If you experience symptoms of severe headache, shortness of breath, nausea, vomiting, itchiness or redness, please present to Lake Macquarie Private Hospital (Private Emergency Department) or John Hunter Hospital (Emergency Department). Please inform the hospital that you have had treatment with Dr Tan-Gore. 


For the first 24 hours post treatment

  • Do not apply pressure on the injection areas – this may cause migration of the Neurotoxin to unwanted areas

  • No strenuous exercise/extreme heat or cold e.g. ice packs or saunas

  • No swimming

Informed Consent

Contraindications to Neurotoxin Treatment


  • Pregnant or breastfeeding

  • Allergy to cow’s milk protein, neurotoxin, sodium chloride, albumin

  • Infection in the areas of proposed  injection site

  • Neuromuscular disorder/disease, not limited to myasthenia gravis, multiple sclerosis, Parkinson’s disease, amyotrophic lateral sclerosis


Precaution

Patients should reconsider or delay treatment if the following apply


  • Pre-existing cardiovascular disease

  • Seizures

  • Glaucoma

  • Medications such as Aminoglycosides, calcium channel blockers, anticholinesterases

  • Psychological stressors

  • Unrealistic expectations


Common Post Treatment Side Effects:

  • Headache

  • Redness

  • Bruising

  • Swelling

  • Numbness

  • Flu-like symptoms


Conditions of Treatment:

  • May not improve pain symptoms in certain circumstances

  • Difficulty chewing/decreased mouth opening : The action of major muscles of mastication is affected. This usually improves within a few weeks

  • Asymmetrical smile: Caused by inadvertent diffusion of neurotoxin into the surrounding muscles. Resolves within 3-4 weeks

  • Paradoxical Bulging: Caused by compensation of unaffected deep and superficial muscle fibres . Resolves spontaneously. Occasionally requiring further doses to resolve

  • Sunken Cheeks/Sunken Temple area/Sagging: Masseter and Temporalis relaxation may cause overlying skin to sag. Resolves within 3-4 weeks

  • Facial Shape Changes: This may be undesirable for some patients

  • Dry Mouth: Rare
